7K a year 2 hours ago 2 hours ago 2 hours ago The Patient Scheduler/Care Coordinator is responsible for assisting with the coordination and scheduling of care with nurses, patients/caregivers, MDs and other healthcare providers.
He/She also maintains log books associated with productivity of the agency, and ensure credential compliance of all its employees.
He/She reports directly to the VP of Nursing Services/Administrator, who reports to the CEO.
Requirements:
Has at least one year's experience in any customer relations field of work / office environment related to clinical care of patients.
Excellent interpersonal verbal and written skills in relations with office peers, field nurses, caregivers, Physician's, and other related entities.
Demonstrates excellent organizational, prioritizing, and time management abilities.
Be able to multitask and take direction from others well.
Thorough and detail oriented.
Ability to manage office files-paper and computerized and log books.
Ability to work independently without constant supervision.
Be able to read, write and comprehend English.
Must be able to read 12 point or larger type.
Must be able to hear adequately with no more than an amplifier on the phone and to speak in a manner understood by most persons.
Responsibilities:
Demonstrate professionalism and concern for clients and their caregivers, as well as all co-workers, at all times.
Attend all meetings as required.
Project a concerned, professional attitude with all departmental and interdepartmental personnel.
Work to develop a positive rapport with all staff members and community resources affiliated with home health care services.
Scanning and logging archived paper documentation into agency's storage drive, as needed.
Develop and implement systems to ensure organizational and timely receipt of current credentials from employees monthly.
Establish mutual goal setting and achievement standards.
Send information out to the Patient's homes for home chart.
Make birthday cards for patients and employees, as well as anniversary cards for employees monthly.
Assist with updating and maintaining productivity logs, and other logs as directed by Supervisor.
Provide support to staff and employees in the field as needed.
Answering phones and transfer to appropriate team member based on needs.
This includes covering phones for other departments, if needed.
Order medical supplies as needed.
Perform duties as outlined in policies and procedures.
Perform any other duties as assigned by VP of Nursing Services, Administrator, CEO, or CFO and work closely with them when needed.
Assists with making manuals for trainings and education classes.
Send out Team Conferences/Patient Summary reports to physicians, caregivers, and staff accordingly, and ensure receipt.
On-going staffing of existing patients, staffing new patients, making follow-up calls/communications with nurses/caregivers as needed.
Participate in the Staffing On-Call Rotation at least 2 full weeks each month, or as directed by the VP of Nursing Services.
Mass mail outs or electronic messages as directed by Supervisor.
Maintain high visibility and availability while in the office.
Participates in obtaining required credential documents from field staff.
Participates with creating new orientation packets for all nurses/staff, as well as admission packets for new patients Create and manage making new patient start-up boxes and home chart for new patients.
Answering incoming calls and directing them to the appropriate personnel.
Participate in the rotation of covering the front desk and answering phones for other departments when needed.
Any other duty or responsibility deemed by the VP of Nursing Services, CEO or CFO.
